2010/2011 Forms
Form Due Dates for 2010/2011 Are as Follows.
Forms must be turned in as teams individual forms not accepted.
Uniform Requests go through DMTeam and are due by June 25th see Uniform Order Process below
Commitment letters for all teams except U15 and above girls can be turned in starting July 1 and are Due by July 15.
For U15 and above girls forms can be turned starting Oct 1 and Due by Oct 15
This is to ensure teams get register(Ensure we have Whole team and registration Fees one Check per team)
Coaches\Managers Registration Please provide a $30 check for each coach assigned to your team as part of your team registration check
